microchip has been implanted in your pet, your veterinarian
will provide you with a HomeAgain®
Lost Pet Recovery Service enrollment
form. In order to enroll your pet in the database, complete and mail this form to

Your veterinarian will also provide you with a special yellow tag to attach to your
pet's collar. This promptly alerts any animal shelter or veterinarian that your
pet has a microchip and is enrolled in the HomeAgain®
